Medical RN Case Manager Garland, Texas

Summary:

The RN Case Manager will coordinate the overall care of a caseload of clients including supervision and management of the multidisciplinary health care team comprised of RN’s, LVN’s, Aides and auxiliary service personnel

Requirements:

*Must currently be licensed as a RN in the state of Texas

*Shall have at least one year experience in the home health industry

*Reliable Transportation

*Willingness to travel within San Antonio and surrounding areas. (e.g. 100 to 300 miles weekly, with variance from week to week)

*Wound Vac experience preferred

*Competent with standard wound care procedures

*Willingness to be on-call after hours weekends at least one week each month (Regular Visits not typically scheduled after hours or on weekend. On-call visits are only done as need in accordance with CMS compliance requirements and care standards)

*General proficiency with using computers

*Shall Have strong documentation and verbal skills to facilitate appropriate communication in varied situations

*Shall have basic understanding of Medicare rules and regulations for home care and Medicare guidelines for home health coverage and compliance.

*Must have Current Texas Drivers License

*Must be able to travel and to perform field visits as needed. Not all sites/locations will be handicapped accessible.

Salary Range:

$50,000 to $65,000 depending on experience and home health knowledge.
